To get a jump on the enrollment process, please complete our pre-enrollment form. This will allow CBA to provide one-on-one assistance to make sure you are ready to enroll.
Get ready to enroll! Please gather the following materials prior to April 13 to speed up the enrollment process:
- Birth certificate
- Vaccine record
- A valid photo ID
- Most recent utility bill (Water, Gas, or Electric) or a current Lease Agreement
